It might be time for some you to be getting ready for more Parent Teacher Conferences. Conferences can be a great opportunity to connect with parents and it’s a great opportunity to share important information about your students with their parents.
Here is a form on dropbox and on TPT a colleague and I modified to use for our Parent Teacher Conferences at the end of 1st Quarter. I shared them with all of our Elementary teachers, and one of them even had the office make them into a triplicate carbon copy form. I felt kind of like a published author when I saw a form I created printed in triplicate. :)
I also found a PTC reminder slip to send home with students letting parents know what time their appointment is.
Here is also a self report card I had the kids complete that was fun to share with parents.

What are your PTC tips? Anything you like to do to make things run smoothly? Offering candy or snacks is always a great idea, right? :)
Help! I can’t get it to open in full page!!! :)
Thanks for the great form!
LOVE these- thanks for sharing!
YEAH!!!! So glad that the student self-reflection helped your students!!! :)
Melissa :)
Help!! I cannot download the PTS form!! I love it!!
<3 it!! Thank you so much for sharing!
thanks for your awesome forms! You just saved me hours of work!
When I originally left a comment I appear to have clicked on the -Notify me
when new comments are added- checkbox and now each time a
comment is added I get 4 emails with the same comment.
There has to be a means you are able to remove me
from that service? Many thanks!
Thank you so much for the available forms. It was quite helpful.
Do you have the Student Self-Reflection sheet that I could edit and include other behaviors. I have used it with lots of my students in my EC classroom and the students have learned and enjoyed it. Thanks so much for sharing!
I am not able to get the Self Reflection sheet, any way I could get it emailed to me?
Thanks!
